We are seeking a dedicated and proactive Multi Agency Homelessness Response Officer to join the Rough Sleeping Team. In this role, you will coordinate and facilitate a multi-stakeholder approach to supporting individuals experiencing homelessness and sleeping rough. You will ensure effective collaboration between statutory services, charities, and community organisations to improve outcomes for those discussed in multi-agency meetings.
We need someone that has the skills required to organise, chair, and facilitate Multi-Disciplinary Team (MDT) meetings, setting strategic direction and ensuring follow-up actions are implemented effectively. You will design and maintain a robust tracking system, oversee data integration across services, and ensure timely and coordinated responses from relevant agencies. Acting as the liaison for all organisations involved in the MDT, you will ensure strong partnership work and seamless information exchange. You will collect, analyse, and manage key service data to enhance case oversight and inform strategic decision-making, producing reports to track service user outcomes and support evidence-based improvements. Additionally, you will reduce administrative burdens on frontline staff by handling record-keeping, scheduling, and follow-up tracking, independently monitor and drive follow-ups on MDT actions, ensuring no service user falls through the gaps, and contribute to the development and implementation of policies and procedures that enhance project delivery and service coordination.
This post is being offered as a fixed term contract or secondment opportunity to LBE staff for 2 years. The continuity of the post after the initial term will depend on our capacity to secure further funds.
